Correct directions and dosage for ipecac!!?

I own a daycare and just bought the mandatory ipecac for my center. On the bottle it had a "peel here" area with the main instructions and dosages. However, the label was crappy and did not peel cleanly and ended up tearing all the information I needed clean off the bottle. So now I'm left with NO instructions on how to use it if I ever have to. If anyone has a bottle around and can give me a summary of the directions i'd appreciate it! I plan on getting another bottle for reference for my staff but just in case, until then any help would be great! Thanks!
